Antonio Medina and Jorge Cerpa were united Friday during a lavish ceremony in the city's Iztapalapa neighborhood.
Mexico City's liberal legislature passed the new civil union law in November. It grants male and female same-sex couples the right to jointly own property and inherit each other's estates.
The country's first same-sex civil union ceremony took place in January in the northern state of Coahuila.
Homosexuality has long been considered taboo in Mexico, which is nearly 90 percent Catholic.
